The Partners in Autism program provides services to clients between the ages of 12-18 and their parents/guardians/caregivers who are facing imminent crisis either in their home, school or community as a result of complex needs and behaviours. These clients, who have a diagnosis of autism, and their parents/guardians/caregivers receive a circle of support through the Partners in Autism program with the assistance of a Lumenus Service Coordinator and a Kerry’s Place Autism consultant. Collaboratively, the team offers service coordination and assists the parents/guardians/caregivers in navigating the system to remove barriers and provide access to the support and services required. The team also supports the clients in enhancing their social, life and community skills. This transitional program works with clients and their parents/guardians/caregivers for up to 6 months.
