The Resource Clinic provides families who are registered under the OAP with the opportunity to meet with an Intake and Family Support Coordinator one-to-one. An Intake and Family Support Coordinator can assist you with navigating and answering questions you have about supports and services available. Both virtual and in-person (at 155 McNicoll Ave., Toronto, ON M2H 2C1. ) appointments are available Please register to schedule an appointment. An Intake and Family Support Worker will call you to confirm your appointment time.
