Our Service Model

Bringing services and communities together to create seamless access to care.

One Connected System of Support

At Lumenus, we recognize that every person’s journey is unique. Many individuals and families access more than one service throughout their care experience, which is why our programs are designed to work together as a connected system of support.

Rather than navigating multiple services on their own, clients benefit from integrated care that brings together expertise across mental health, autism, developmental, crisis, family and community services. Our team helps coordinate care, guide next steps, and connect individuals to the programs and supports that best meet their needs.

Working closely with schools, healthcare providers, childcare organizations and community partners, we ensure care is collaborative, responsive and centered around the whole person.

The result: seamless pathways to support, reduced barriers to care, and better outcomes for the people and families we serve.

This approach helps us fulfill OUR PURPOSE: Create seamless access to services while reducing barriers to care. In this way, we ensure every client has the opportunity to be seen, be heard and to be well. Lumenus understands that many of our service users and clients have unique needs and may want to access more than one area of expertise.
